What is a "Basic 2-5 Revision Request Form?"
A "Basic 2-5 Revision Request Form" is a form that enables a Team Executor to carry-out the individual revisions requested by a member for a serviced Basic 2-5 Webpage Website (or Blog Webpage/Add) SetUp. A Basic 2-5 Webpage Website service includes the setup of two to five webpages; which includes either a Home Page, Service Page, Contact Us Page, About Us Page and/or Blog Page. A Blog Webpage/Add service includes the setup of a Blog Page ONLY and/or the adding of blogs to the setup Blog Page.
If a Basic 2-5 Revision Request service, Basic 2-5 Webpage Website Service and/or a Blog Webpage/Add service is requested during a consultation, the assigned Team Executor will send this link and the PDF form version to the member's email.
For a Basic 2-5 Webpage Website Setup Service, depending upon number of webpage(s) requested and purchased, one to two forms (approximately 21 to 42 revisions respectively) maybe included within your service plan. Included forms are required for submission within 2 weeks subsequent to your T.E. completing your webpage setup service. However, if the two-week period has expired and additional forms are submitted for revisions, additional cost per form will be applied.
There are multiple areas to upload photos throughout this form.

The timeframe for a Team Executor to complete revisions for a Basic 2-5 Webpage Website service depends upon the service period and how many revisions were requested and how much content is changed per member request.
