This form indicates that a current membership and its service plan; which are known to be operative, active and in good standing, will be transferred form one party to another. Once this form is complete, signed and processed the receiver of this transfer is hereby the legal owner of the transferred VisProsper membership; which includes ALL CURRENT & PRIOR TO services and accounts affiliated with the membership.
